Please download, print and fill out the following Patient Registration forms. Doing this prior to your appointment will help speed up our service to you and our other patients. Please bring your insurance card to your first visit. Also bring a list of all medications you are taking.

Please download, print and fill out Part 1 of the "Report of Medical Examination and Vaccination Record" i.e. Form 693. The latest version of this form can be downloaded from the USCIS web site at the link below. You will also find the instructions for filling up the form on the same page. Please do not sign or date the form until instructed to do so by the Civil Surgeon.
