Print out and bring the most recent version of Form I-693, Report of Medical Examination and Vaccination Record, to your medical exam appointment: Fill out Part 1 of Form I-693 but do not sign until the civil surgeon instructs you to do so.

Radhika Jasthi MD is Board Certified in Internal Medicine and is affiliated with the Stamford Hospital. She has been serving the Stamford community since 2003.

Dr. Jasthi is also a Civil Surgeon who is designated by the United States Citizenship and Immigration Service (USCIS) to provide immigration medical check-up for U.S. permanent resident (Green Card) applicants.
